This gatefold illustration shows an underground sewer scene. The overall color palette has slightly more saturated reds in the background with more desaturated grays, blues, and greens in the foreground.
Casting light from the top is a large, pale moon, which reveals a hole in the sewer tunnel. The tunnel is lined with bricks in varying states of decay and cracks. A legion of rats crawl from outside the tunnel through the hole and down the walls, forming a thick pile of rats at the bottom, near the water. More rats swim across the sewer towards the viewer and climb back up into a smaller pipe on the other side. On both sides of the hole are sewer grates with giant rats trying to claw out and gnawing on the metal bars. There is a reflection of the moon in the sewer water. Piles of sludge and slime are scattered all over the walls and floor of the tunnel.
To the left of the illustration is a throne made of trash, stuck together with sludge. There are bits of cans, plastic, and even rat corpses within the slude. A grinning man sits on the throne, wearing a desaturated green-yellow tie-dye shirt. He is wearing a golden crown, as per his position as the Rat King. Next to him is a woman with dark, long hair and wearing a dark dress. She is touching his arm while he runs his fingers through her hair.
